    Spravato vs. IV Ketamine

    What’s the difference really?

    Spravato and IV ketamine are two different ways to receive ketamine-based treatment. Spravato is an FDA-approved nasal spray that’s often covered by insurance, but your body absorbs less of the medication. IV ketamine is delivered directly into your bloodstream, providing a more predictable, controlled experience. Both options can be effective; the right fit depends on your symptoms, goals, and whether you prefer an insurance-covered nasal spray or the precision of IV infusions.

    What is bioavailability?

    Bioavailability describes how much of a medication your body actually absorbs and can use. For SPRAVATO, the esketamine nasal spray, the average bioavailability is about 48%, meaning only about half of the dose enters your bloodstream after administration. That’s significantly less than an IV ketamine infusion, which delivers the medication directly into your bloodstream for near-100% bioavailability. Because IV delivery bypasses absorption barriers, it ensures a more predictable, consistent, and dosing.

    Will I see results from Spravato?

    Studies show that Spravato helps a significant number of patients with treatment-resistant depression: response rates (meaning a meaningful reduction in symptoms) fall roughly in the 50-60% range. This means 50-60% of patients find relief.

    If you’re looking for substantial relief, many studies report 80% of patients who receive IV ketamine will experience this often within hours or days and complete remission rates reach a notable portion of patients. 

     

    What have the experts seen?

    A study conducted in 2023 that was presented at the American Psychiatric Association (APA) annual meeting and published in the New England Journal of Medicine showed IV Ketamine matching the ‘gold standard’ in major depression treatment. The ketamine participants had a 14.2% treatment difference in the trial when compared to ECT which has been the previous gold standard for 80 years.
