Northwest Ketamine Clinics (“NWKC,” “we,” “us”) operates ketamine treatment clinics in Bellevue, Seattle, and Tacoma, Washington, and the website at nwketamineclinics.com. Your privacy matters to us, and this policy explains how we handle information collected through our website.
This policy and your medical record are two different things
This policy covers information collected through our website: what you type into a form, what your browser sends us when you visit, and what our vendors see on our behalf.
Your medical record is different. Once you become a patient, the health information we create and keep about your care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). How we may use and disclose it, and the rights you have over it, are described in our Notice of Privacy Practices, not here. If the two documents ever appear to conflict about your health information, the Notice of Privacy Practices governs.
Information we collect
Information you give us
When you fill out a form on our site, request a consultation, join a waitlist, or contact us, we collect what you choose to enter. Depending on the form, that can include your name, phone number, email address, city, insurance information, the service you’re interested in, and anything you write in a free-text field.
Please don’t use our website forms to send us detailed health information. A form is a good way to ask us to call you. It isn’t a secure channel for your medical history. Call us at (206) 309-2299 and we’ll take that conversation somewhere appropriate.
Information collected automatically
When you visit, our site and our analytics provider may receive your IP address, device and browser type, operating system, referring page, the pages you view, how long you stay, and general location inferred from your IP address. We use this to understand how the site is used and to improve it.
Cookies and similar technologies
We use cookies and comparable technologies for site functionality and analytics. Most browsers let you refuse or delete cookies through their settings. Refusing cookies may affect how parts of the site work.
Third parties who process information for us or receive it
Our website relies on outside services. The ones that can receive your information are:
- Form and scheduling provider. Our consultation and contact forms are hosted and processed by a third-party form provider. What you submit passes through and is stored on their systems.
- Analytics and tag management. We use Google Analytics through Google Tag Manager to measure site usage. Google receives the automatically collected information described above.
- Supplement storefront. Our supplements page may connect to a third-party dispensary platform. If you create an account or place an order there, that company collects your information under its own privacy policy and terms, and the transaction is with them, not with us.
- Hosting and infrastructure. Our website host stores site data and server logs on our behalf.
- Social platforms. Our site links to our Facebook and Instagram pages. Those platforms may collect information when you interact with content there.
We don’t sell your personal information, and we don’t share it with third parties for their own advertising purposes.
How we use information
We use the information described above to respond to your inquiry, schedule and confirm appointments, provide and improve our services and our website, communicate with you about your care and about services you asked about, meet our legal and professional obligations, and detect and prevent fraud and misuse.
Text messages
If you give us your mobile number, we may text you about appointments, scheduling, and your care. Message and data rates may apply. You can opt out at any time by replying STOP. We do not share mobile numbers or text-messaging consent with third parties or affiliates for their marketing or promotional purposes.
How long we keep information
We keep website information only as long as we need it for the purpose it was collected, and then we delete it or strip the details that identify you. Where a law, accounting rule, or professional obligation sets a retention period, we follow that instead. Medical records are retained under applicable law and our record-retention practices, which are separate from this website privacy policy.
Security
We protect the information we hold using commercially reasonable safeguards against loss, theft, and unauthorized access, disclosure, copying, use, and modification. No method of transmission or storage over the internet is completely secure, and we can’t guarantee absolute security. Where you hold an account with us or with one of our vendors, keeping your password secure is your responsibility.
Children
Our website is not directed at children under 13, and we don’t knowingly collect personal information from them through it. Our clinical services have their own age criteria, and where we treat patients under 18, their care and their records are handled under Washington law and the consent rules that apply to their age and treatment, as described in our Notice of Privacy Practices.
Your choices and your rights
Providing information is your choice
You don’t have to give us personal information, but if you don’t, we may not be able to answer your question or schedule your visit.
Access and correction
You may ask what personal information we hold about you and ask us to correct anything inaccurate, incomplete, or out of date. For information in your medical record, use the process in our Notice of Privacy Practices, which sets out your HIPAA rights and how to exercise them.
Marketing
If you agreed to receive marketing from us, you can withdraw that at any time using the contact details below or the unsubscribe link in any email.
No retaliation
We won’t deny you goods or services, charge you a different price, or give you a lower quality of service because you exercised a privacy right.
Complaints
If you believe we’ve mishandled your information, contact us using the details below with the specifics. We’ll investigate and respond in writing. You may also contact a regulatory authority, including the Washington State Attorney General’s Office, and for health information the U.S. Department of Health and Human Services Office for Civil Rights. We won’t retaliate against you for filing a complaint.
Data breaches
If a breach affecting your information occurs, we’ll notify you as required by applicable law.
Washington residents
Washington’s My Health My Data Act gives Washington consumers specific rights over consumer health data that falls outside HIPAA’s protection. Those rights, and the disclosures that Act requires, are described in our separate Consumer Health Data Privacy Policy, linked from our homepage.
Additional disclosures for other U.S. state privacy laws
Residents of California, Colorado, Connecticut, Delaware, Florida, Oregon, Texas, Utah, and Virginia may have additional rights under their state’s privacy law, including rights to know what personal information we collect, to request deletion, and to correct inaccuracies. To exercise any of them, contact us using the details below with “Privacy Rights Request” in the subject line, and include enough information for us to identify you and locate your records. We’ll respond within the time your state’s law allows.
Do Not Track
Some browsers offer a “Do Not Track” setting. We don’t currently respond to those signals.
California
Under California Civil Code Section 1798.83, California residents whose relationship with us is primarily personal, family, or household may ask once per calendar year about personal information we disclosed to other organizations for their direct marketing purposes. Send the request to the address below with “Request for California privacy information” in the subject line.
In the preceding 12 months, we have collected these categories of personal information: identifiers such as name, email address, phone number, and IP address; customer records such as billing information; internet and network activity such as pages viewed and interactions with our site; general geolocation inferred from IP address; and, from job applicants, employment and education information. We collect them for the purposes described in “How we use information” above.
Links to other websites
Our site links to websites we don’t operate. We don’t control their content or their privacy practices and aren’t responsible for them. Read their policies before giving them your information. This policy stops applying once you leave our site.
Business transfers
If we’re acquired, merge, or transfer substantially all of our assets, information we hold may be among the assets transferred. Any acquirer would be required to honor this policy with respect to information transferred, to the extent applicable law allows.
Changes to this policy
We may update this policy to reflect changes in our practices or in the law. Updates will be posted at this same address with a revised “Last updated” date. Where the law requires it, we’ll obtain your consent or give you the chance to opt in or out of a new use of your information.
